Minnesota Family Planning Program

4.1.4.2 Renewals (Archive)

The eligibility period for the Minnesota Family Planning Program (MFPP) is 12 months. Enrollees must submit the Minnesota Family Planning Program Renewal (DHS-5440) before the first day of the month coverage is to end.
