Clients are required to report changes that may affect eligibility. This section provides more information regarding this topic.

How Changes May be Reported

Clients may report changes by:

l  Phone.

l  Mail.

l  In person.

l  Using a required scheduled reporting form.

Clients may use the Change Report Form (DHS-4796) to report changes.

Changes may also be discovered in other ways, such as:

l  Changes reported by another person or agency.

l  Changes reported by an enrollee to another program. For example, enrollees may report a change to Food Support staff that also affects their MinnesotaCare eligibility.

l  Information reported by computer matches (such as IEVS).

l  Upcoming or potential changes that the agency has been tracking.

Required Changes to Report

Clients must report changes that may affect their eligibility within 10 days after the date of the change, unless noted otherwise.

Changes that clients must report include, but are not limited to:

l  Household composition, including household members moving in or out, births, deaths and marriages.

l  Access to health insurance or Medicare.

l  Pregnancy.

l  Address.

l  Employment, including stopping, starting or changing employment; starting or stopping a business; and changes in hours or earnings.

l  Assets.

l  Unearned income.

Processing a Change

When changes in circumstances are reported or discovered, take the following steps:

1. Determine if more information is needed. If yes, request the information.

2. Determine if the change affects eligibility for any household member.

3. Determine if the change affects the spenddown or premium amount for any household member.

4. Apply needed changes according to program rules.

l  If no action is taken, document the circumstances in the case record.

l  Track the change if it will require future action.
