Minnesota Health Care Programs Membership Card (Archive)

The Department of Human Services issues a Minnesota Health Care Programs (MHCP) membership card to each eligible enrollee. MHCP membership cards for new enrollees (as well as replacement cards) are generated each week by MMIS.

Note:  Managed care enrollees also receive an identification card from their managed care organization (MCO).

Using the MHCP Card

Enrollees must present their MHCP card along with other third party liability information to all health care providers. They must be eligible during the time of service for the provider to be paid. While the card does not verify eligibility, it does include:

l  The enrollee's PMI number, name, birth date, and gender.

l  The metro and 1-800 telephone numbers for enrollees to call with questions about medical services they have received.

l  The information a health care provider needs to contact the DHS Eligibility Verification System (EVS).

l  The DHS address to which providers should submit fee-for-service claims.

Card Not Yet Received

If enrollees need health care services before they receive their MHCP card, providers can still use the PMI number for each approved person to verify eligibility through EVS.

l  The PMI numbers are shown on initial approval notices.

l  You may also verify the PMI number and eligibility dates on request. The client consented to the release of this information by signing the application.

Replacement Cards

Issue replacement identification cards at no charge to the enrollee for:

l  Lost, destroyed, damaged, or stolen cards, or

l  Corrections to name, gender, or date of birth.

There is no limit on the number of replacement cards. See the MMIS User Manual (Health Care ID Cards) for information on how to issue replacement cards.
