A client may apply for Minnesota Health Care Program (MHCP) coverage using a variety of methods.
Clients may request coverage:
l By phone.
Send an application form no later than the next work day to clients who inquire by phone.
n Send the application form appropriate for the client’s situation.
n Encourage the client to return the application as soon as possible.
n Explain that the client may choose to complete the application at the agency.
l In person.
The client may submit a written request at the time they are in the office. Provide the client with an application on the same day they are requesting coverage.
l In writing.
To establish a date of application prior to submitting an actual application form a client must provide at least the following information in writing:
n Name.
n Address.
n Date.
n Signature.
Mail the application no later than the next work day.
Note: A request that is received without the minimum information, including from providers applying on a client’s behalf, cannot set the date of application. Return the request informing the client that the request is not valid because the minimum required information is missing.
A submitted application is also considered a request for coverage unless it does not include the minimum information.
A completed application must be submitted within the program processing period or the request for coverage will be denied.
Clients must provide a completed application form, required information and verification to an enrollment site in order to have their eligibility determined. Clients may submit the completed application form to the enrollment site in any of the following ways:
l Mail.
l Fax.
n A faxed application may be used to set the date of application and determine initial eligibility.
n The original application must be received by the enrollment site within 30 days of the date the application was faxed. If not received within this time frame eligibility may be closed or the application denied with 10-day notice.
l Hand-deliver to the enrollment site.
Clients may request help with completing the application process, which includes help filling out the application form and contacting third parties to get needed information and/or verifications.
Provide clients who appear to be having difficulty with the application process with information on where and how to receive help.
Clients may receive help from MinnesotaCare Operations or their local county agency.
