Signatures from specific people are required for all renewals - annual, monthly and six-month.

Renewal Signature Requirements
Signatures are required from all enrollees age 18 and older at each renewal, or for those who are age 18 and over who are applying for coverage on the renewal.
Note: Accept the signature of a designated authorized representative.
l Pend renewals for applicants age 18 and over who do not sign the renewal or who do not have an authorized representative sign.
l Require the client’s and the authorized representative’s signature in order to complete the designation if a household designates an authorized representative at renewal.
l Send a photocopy of the renewal form with the Request for Signature Letter (DHS-3555) to obtain the missing signatures.
l Eligibility will end for enrollees who have not provided required signatures by the renewal month.
Note: For MinnesotaCare (MCRE), eligibility will close for enrollees whose eligibility depends on that of the person who failed to sign under the All or Nothing Rule. See All or Nothing Rule.
