09.15ar1 - Obtaining Verifications (Archive) |
Archived: May 1, 2007 |
This section details how to assist clients in obtaining mandatory verifications.
State Verification and Exchange System (SVES).
Providing verification is the responsibility of the client.
Assist clients in obtaining verification if the client is unable to provide it.
l Obtain the client’s written consent before contacting third parties for verification. The signed release must identify the source and the specific information to be requested. See Release Forms.
l If neither the client nor the agency is able to obtain outside verification, accept the client’s written statement attesting to the correctness of the information.
Example:
Mortimer and Matilda apply for health care for themselves and their children. They report two vehicles, one of which is determined to be excluded. The value of the non-excluded second vehicle will be counted in the respective asset total. The worker contacted Mortimer and Matilda regarding the value of the second vehicle. They were unsure of the value.
Action:
The worker helped them access the value by looking it up in the NADA book. No release of information was needed for this.
The following release of information forms are available for use in helping the worker help the client obtain required verification:
l General Consent/Authorization for Release of Information (DHS-3549).
This release form is used to gather information and the authorization/consent of an applicant for food support, cash assistance, health care, child support services, or a license to provide services
l General Authorization for Release of Information (DHS-2243A).

l Consent for Release of Employment Information (DHS-2146).

State Verification and Exchange System (SVES)
The State Verification and Exchange System (SVES) is an interface provided to workers to query online with the Social Security Administration (SSA). This interface is used to:
l Verify Social Security Numbers in a monthly batch job.
l Initiate some IEVS matches. See IEVS for more information.
l Provide verification of SSI and RSDI payment information.
The SVES interface does not provide information on which beneficiary obligations, such as child support, are deducted from the gross RSDI amount.
