People may withdraw their application for health care if the application has not been pended, approved or denied.
Requests to withdraw the application can be made in any of the following ways:
l Orally.
l By written statement.
l By writing "withdrawn" on the application form and initialing it.
A notice must be sent to confirm the withdrawal.
