The purpose of the Long-Term Care Consultation (LTCC) is to prevent inappropriate nursing or certified boarding care facility placements. Everyone seeking admission to a nursing or certified boarding care facility must be screened before admission regardless of pay or source.
LTCC is a screening program that determines an individual’s level of care by assessing a client’s health and psychosocial needs.
The screening process is defined by the Aging Division of DHS.
Refer people to the county LTCC team if they are:
l Seeking admission to a nursing home or certified boarding care facility.
l Applying for:
n Long-Term Care (LTC) coverage.
n Elderly Waiver (EW).
n Alternative Care (AC).
n Community Alternatives for Disabled Individuals (CADI).
n Community Alternative Care (CAC).
n Traumatic Brain Injury (TBI).
People may choose between community-based and facility care.
Note: MA will not pay for nursing or certified boarding care for people who choose to enter a facility when the county LTCC team has determined that the person does not meet nursing/boarding facility level of care criteria.
The county LTCC team will determine whether the person needs a nursing home or certified boarding care facility level of care by assessing the person's health and psychosocial needs.
l The team must inform people of available alternatives to institutionalization and recommend either a facility placement or community-based care.
l The LTCC team enters risk status on the LTC Screening Document.
Verify that MA applicants or enrollees have either had or are exempt from LTCC screening before admission.
Note: MA will not pay for nursing facility or certified boarding care services received before the LTCC date indicated on the LTC Screening Document.
People who refuse long term care consultation or who are not determined to be at risk or in need of a nursing or certified boarding facility level of care may receive certain MA services such as home care, but may not receive:
l MA nursing or certified boarding care facility services.
l Waivered services, designed to provide community alternatives to facility care, provided under:
n EW.
n CADI.
n TBI-nursing facility waiver or Neurobehavioral waiver.
n CAC.
