Automatic Withdrawal Plan (AWP) (Archive)

Enrollees may choose to have MinnesotaCare or MA-EPD premiums deducted automatically from their bank accounts.

l  The MinnesotaCare Information form (DHS-3104) and the Automatic Withdrawal Plan (DHS-3389) contain information on AWP.

l  To sign up for automatic withdrawal, enrollees must complete the form on the Automatic Withdrawal Plan (DHS-3389) and send it with a voided check or deposit slip to:

DHS Automatic Withdrawal Plan,

PO Box 64834,

St. Paul, MN 55164-0834.

Fax:  (651) 431-7485.

l  The payment is taken from the account on the 10th of each month, or the next business day if the 10th is on a weekend or holiday.

l  Automatic withdrawal begins approximately 60 days after the enrollee submits the form.

l  DHS Financial Operations sends the enrollee a confirmation letter indicating the date the AWP will begin.

l  Enrollees may end AWP at any time by calling DHS Financial Management at (888) 234-1321 or (651) 431-3362.

l  DHS Financial Operations will stop AWP when they receive a request or if there is no bill generated.

l  Once AWP has been stopped, enrollees must pay premiums using another payment method until they sign up for AWP again.

l  Clients with questions may contact DHS Financial Management at (888) 234-1321 or (651) 431-3362.
