Minnesota Disability Health Options for people with physical disabilities (MnDHO-PD) is a voluntary managed health care program available to some Medical Assistance (MA) enrollees who are certified blind or disabled through the Social Security Administration (SSA) or the State Medical Review Team (SMRT). The program is administered by DHS, UCare Minnesota (through their UCare Complete product), and AXIS Healthcare (a care management organization for people with physical disabilities).
Note: A pilot program for people with development disabilities (MnDHO-DD) is underway in Carver, Hennepin, and Scott counties.

Coordination With Other Programs.
MnDHO provides flexible alternative services beyond the scope of MA and waiver services. A key feature of MnDHO is that all members are assigned a health coordinator, typically a nurse, who helps them navigate the health care system and get the services they need.
To be eligible for MnDHO, people must meet all of the following:
l Have a physical disability.
l Be at least age 18 and under age 65.
l Live in a county participating in MnDHO (Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, or Washington).
l Be enrolled in MA (including MA for Employed Persons with Disabilities, or MA-EPD).

MA enrollees who are also enrolled in
will receive both MA and Medicare services through MnDHO.People who receive home and community-based services through the Community Alternatives for Disabled Individuals (CADI) or the Traumatic Brain Injury (TBI) waiver programs are eligible to enroll in MnDHO. CADI and TBI participants who choose to enroll in MnDHO will still get the waiver services but will receive case management services from a health coordinator at AXIS Healthcare instead of a county case manager.
