4.1.1.4.1 Change in Circumstances

Minnesota Family Planning Program (MFPP) enrollees must report changes that affect eligibility. Changes that people are required to report include, but are not limited to:

Household composition, including members moving in or out, births, deaths, and marriages
Household tax filing status and tax dependent status
Income
Immigration status
Losing Minnesota state residency
Moving into an intermediate care facility for mental disease (IMD) or state prison, county detention facility or city jail
Pregnancy

Reporting Changes

Applicants and enrollees must report changes to DHS agency within 10 days. They may report changes via:

Phone
Mail
In person
Using a renewal form
Using the Change Report Form (DHS-4796)

Changes are discovered in other ways, such as:

Changes reported by another person or agency
Changes reported by an enrollee to another program, such as Supplemental Nutrition Assistance Program (SNAP)
Information reported by electronic matches
Upcoming or potential changes that the agency has been tracking

MFPP enrollees must reconcile inconsistent information with DHS.

Eligibility Redetermination

When an enrollee reports a change in circumstances, eligibility must be redetermined with the new information. If the change results in no change in eligibility, no action needs to be taken by the agency.

If the change results in ineligibility for MFPP, coverage ends the last day of the month for which advance notice can be given. Generally, 10-day advanced notice is required to close coverage for the enrollee. See Section 1.3.1.5 Minnesota Health Care Programs (MHCP) Notices for more information.
